CARING FOR CANCER PATIENTS WITH XEROSTOMIA AT THE PHARMACY

Xerostomia is one of the main side effects of head and neck radiation therapy. Cancer patients can get advice and solutions at the pharmacy to combat the symptoms of this condition and thus improve their quality of life.


According to the World Health Organization, cancer is one of the leading causes of morbidity and mortality worldwide, and new cases are expected to increase by 70% over the next 20 years. 

Approximately 30% of cancer–related deaths are due to five behavioural and dietary risk factors: high body mass index, reduced intake of fruits and vegetables, lack of physical activity, smoking and drinking. 

Of these, smoking is the most important risk factor and accounts for over 20% of deaths from cancer in general and about 70% of deaths from lung cancer worldwide.  

Together with alcohol consumption, smoking is one of the factors involved in mutations occurring in the DNA of keratinocytes, giving rise to oral cancer which ranks among the 10 most common types of cancer worldwide. 

Malignant tumours of the oral cavity account for 4% of all malignant tumours of the body, and of those, 90% correspond to oral squamous cell carcinoma, the most common of cancers of the head and neck. 

Treatment for cancer may include surgery, radiation therapy, chemotherapy or a combination of all these. Some of these treatments, such as chemotherapy or head and neck radiation therapy, may cause certain side effects and various oral and dental complications. The most common side effect caused by head and neck radiation therapy in the oral cavity is xerostomia.   

Advice and solutions can be provided at the pharmacy to improve the quality of life of cancer patients, including recommendations for the treatment of xerostomia. 

XEROSTOMIA

Xerostomia is the subjective sensation of having a dry mouth and is usually due to the decrease or absence of saliva caused by salivary gland malfunction. 

Patients who receive head and neck radiation therapy may have decreased salivary flow because if the radiation field includes the mouth it may damage the salivary glands responsible for the production of saliva. 

Symptoms usually begin within one to two weeks from the start of radiation treatment, and severity depends directly on the amount of radiation received. These symptoms may include difficulty speaking or swallowing, pain when swallowing, cracked and red tongue, dry palate, loss of lustre of the gums and peeling lips. 

Xerostomia sometimes favours the emergence of other oral complications —such as tooth decay, dysphagia, changes in taste perception, glossodynia, infections or halitosis— in patients undergoing head and neck radiation therapy, as damage to the salivary glands causes changes in the volume, consistency and pH of the saliva secreted. 

That is why it is essential for patients to take the necessary measures to combat xerostomia symptoms, to therefore improve their quality of life and avoid potential complications therefrom. The treatment of xerostomia is symptomatic and is aimed at soothing and relieving dry mouth symptoms. Pharmacists can provide cancer patients suffering from xerostomia with general recommendations to help relieve their symptoms:

  • Stop smoking.
  • Drink water often throughout the day.
  • Avoid long periods between meals.
  • Moisten lips, and if necessary, use lip balms.
  • Cut down on dry, spicy, sticky and sugary foods, and eat more cured cheeses, fruits and vegetables.
  • Avoid products with caffeine or alcohol.
  • Keep strict oral hygiene.
  • Get regular dental check-ups.

  • Long lasting moisturising gel for topical application containing betaine, aloe vera, xylitol and sodium fluoride, used when greater moisturisation of the oral tissues is needed.

The STIMULATING range offers several product forms which are easy to apply throughout the day:

  • Chewing gum.
  • Tablets.
  • Spray.

All of these contain xylitol, fluoride and malic acid. Malic acid stimulates salivary secretion without resulting erosive to the dental enamel.
