“DISSEMINATING MESSAGES ON HEART-HEALTHY LIFESTYLES THROUGH THE PHARMACY CAN HELP PREVENT CARDIOVASCULAR AND PERIODONTAL DISEASES”

Renowned cardiologist, Dr. Juan José Gómez Doblas, tells us in this interview about the cardiovascular health of Spaniards and the relationship between cardiovascular disease and periodontal health. In addition, he offers tips to promote cardiovascular health from the pharmacy. 

Dr. Juan José Gómez Doblas, a specialist in cardiology, family and community medicine and Chairman of the Andalusian Society of Cardiology (SAC), has impressive credentials that make him one of the most prestigious experts in his field. He is Director of the Heart Area at Xanit International Hospital, Head of the Cardiology Unit and Director of the UGC Heart Unit at the Virgen de la Victoria University Teaching Hospital, Research Fellow at the Cardiology Department of Mount Sinai Medical Center, USA, and member of the Executive Committee of the Spanish Society of Cardiology Research Agency. He is also on the Spanish Ministry of Health and Consumer Affairs’ Expert Committee for the monitoring plan of the PICI- Plan Integral de la Cardiopatía Isquémica (Integrated Plan for Ischaemic Cardiopathy).  

How would you define Spaniards’ cardiovascular health in general? 

Overall we can say that the cardiovascular health of the Spanish population has improved every year over the last decade, in parallel to what is happening in neighbouring countries. For the first time in Spain, mortality due to diseases of the circulatory system has fallen below 30%. This slight improvement may be attributed to the improved control of certain risk factors such as smoking and to the therapeutic advances available to us. However, we are far from being satisfied and must continue to reinforce our efforts to improve these figures. On one hand, mortality from cardiovascular diseases is very high in women. On the other, although there have been improvements in the detection and control of some cardiovascular risk factors, in other cases there is a clear upturn, as are the cases of obesity and diabetes. 

And what is the outlook for the future? 

The outlook is that if we get the population to modify their lifestyles progressively in line with what has happened over recent years, we will be able to continue reducing mortality from cardiovascular disease. But this does not mean that we will have fewer cases of heart disease, since the progressive ageing of the population on the one hand and therapeutic advances on the other result in lower mortality, but also greater morbidity. That is, we will have more patients with cardiovascular disease because they live longer and fewer die. 

How can poor periodontal health affect cardiovascular health? 

Periodontal and cardiovascular diseases share potent risk factors such as diabetes and smoking. For this reason alone, there is a close relationship between both diseases. But additionally, periodontal disease is a chronic inflammatory disease of infectious origin which has an impact on other inflammatory diseases including cardiovascular disease. Harbouring chronic inflammation in the mouth enhances inflammatory mechanisms in other organs such as in the arteries. 

How about vice-versa? 

The effect of systemic disease on periodontal health is clearly seen in the case of diabetes. The risk of periodontal disease is multiplied threefold in diabetic patients. Likewise, patients with cardiovascular disease have a higher incidence of risk factors for periodontal disease. Diagnosing and treating periodontal disease is just one more facet in the treatment and prevention of cardiovascular disease. 

What can we do to improve our cardiovascular health? 

Healthy habits recommended to prevent cardiovascular disease are well known and relatively simple. I would focus mainly on the following: 

1. Don’t smoke.

2. Exercise at least 30 minutes three or four times a week.

3. Avoid pre-cooked meals, and keep to a Mediterranean diet.

4. Replace fast food with fruits and vegetables.

5. Lose weight. 

What practical advice would you give to prevent cardiovascular disease? 

The first step is to know what your cardiovascular risk level is by regularly checking the strongest cardiovascular risk factors. For example, monitor your blood pressure, cholesterol and blood glucose on a regular basis; keep to a strictly healthy diet and avoid a sedentary lifestyle; avoid drinking alcohol and smoking, and if you have any cardiovascular risk factors, follow your doctor’s recommendations with regard to control and treatment. 

Are there any intervention studies on the relationship between periodontal and cardiovascular health currently under way?  

There is moderate evidence that the treatment of periodontal disease reduces systemic inflammation and improves arterial function. However, there are currently no adequate randomised trials on clinical events in primary prevention, which is probably because of the difficulty in standardising the type of treatment applied in each case. 

What are hospital cardiac rehabilitation units?  

Cardiac rehabilitation is a set of multidisciplinary measures aimed at increasing the functional capacity of patients with cardiac problems through physical exercise, correcting cardiovascular risk factors, improving the patient's psychological condition, cultivating knowledge of the disease and collaborating in social and occupational reinsertion. In short, it aims to modify the patient’s lifestyle, for life, trying to achieve the main objectives, which are to improve quality of life and to reduce morbidity and mortality. Cardiac rehabilitation is an integral form of prevention that involves professionals from different fields (cardiologists, rehabilitation specialists, nurses, physiotherapists, psychologists, urologists, endocrinologists, etc.), and we believe that dentists should also be included.  

What can be done in these units to promote oral health?  

First of all, patients can be made aware of the importance of periodontitis as a risk factor for cardiovascular disease. Secondly, the oral health condition of patients undergoing rehabilitation can be checked. And, should periodontal disease be detected, patients can then be referred to a dentist for appropriate treatment. 

WHAT CAN PHARMACISTS DO TO PROMOTE CARDIOVASCULAR HEALTH FROM THE PHARMACY? 

Pharmacies are potential health providers, as they are easily accessible to the members of society, whether they are sick or healthy. They are usually the patient's first choice for seeking information on health-related issues. Their involvement in the recognition of cardiovascular risk factors and in the diffusion of clear messages on heart-healthy lifestyles can provide an additional mechanism in the prevention of cardiovascular and periodontal diseases.
